Biofiller, PLLA and exosomes all sit within regenerative aesthetics — but they do not do the same thing.
This guide explains how they differ, what each treatment is designed to achieve and how we decide which may be appropriate.
01What Is a Biostimulator?
A biostimulator is a treatment designed to encourage biological processes associated with collagen production, tissue quality, hydration or regeneration. Unlike traditional dermal filler, which primarily provides immediate structural support or volume, many biostimulatory treatments rely on a gradual response from your own tissues.
Depending on the treatment, the goal may be improved skin quality, better support, gradual collagen stimulation, softer volume restoration, improved hydration, more resilient-looking skin — or a combination of these.
THE RIGHT BIOSTIMULATOR DEPENDS ON WHAT HAS CHANGED — AND WHAT YOU WANT TO IMPROVE.
02Why Does Collagen Matter?
Collagen forms part of the extracellular matrix that supports skin structure. With age, multiple processes contribute to changes in skin thickness, elasticity, facial support, soft-tissue quality and volume distribution.

The aim of biostimulatory treatment is not to “reverse ageing.” It is to support tissue quality and structure in a measured way.
03Autologous Biofiller
Beyond Skin Biofiller is an autologous treatment prepared from the patient’s own blood-derived material — soft-tissue support created from your own biology. It is designed for patients seeking subtle support, natural-looking rejuvenation and improved skin quality, rather than a dramatic filler effect.
Potential areas — only where clinically appropriate — include the under-eye/periorbital region, temples, cheeks, mid-face, nasolabial region, perioral area, marionette area, chin and jawline. Your face is assessed as a whole, not treated from a checklist.
A course of three treatments may be recommended depending on tissue quality, age-related change, areas treated, response and individual goals. A specific result is never guaranteed after three sessions.
04Julaine / PLLA
Julaine is a poly-L-lactic acid biostimulator — the PLLA product confirmed for use at Beyond Skin UK. PLLA is designed to stimulate fibroblast activity and progressive collagen formation over time.
Unlike traditional dermal filler, the visible effect develops gradually. It is not conventional filler, and we never present it as such or promise instant volume. Potential areas may include the cheeks, temples, lateral face, jawline and selected areas of volume loss — only where appropriate.
Sculptra is another PLLA biostimulator in the same category. It is mentioned here only as a recognisable reference for patients who may have heard of PLLA before — formulations, particle characteristics, preparation and treatment protocols may differ. Beyond Skin UK uses Julaine.

05Exosomes
Exosomes are small extracellular vesicles involved in cell-to-cell signalling. In aesthetic practice they may be used as part of regenerative skin protocols — particularly alongside procedures such as microneedling where appropriate.
Potential goals include skin-quality support, recovery support, radiance, texture, hydration and post-procedure support. Exosomes do not contain living stem cells, are never described as a “stem cell facial,” and are never claimed to treat disease.
The exact exosome product used at Beyond Skin UK — its manufacturer, source, purification, regulatory status, intended use, application method and supporting evidence — is published here once confirmed following clinical review, and discussed at your consultation.

08Can They Be Combined?
Sometimes — and often to good effect. Exosomes may sit alongside microneedling as part of a skin-quality plan. Biofiller and PLLA can occasionally sit within one broader plan, and biostimulators are sometimes staged alongside dermal filler.
Whether combining treatments is appropriate is a clinical decision made at your consultation — never applied automatically.
09Black and Brown Skin
Regenerative aesthetics should not be based on one universal facial template. Assessment should respect ethnic facial anatomy, natural mid-face projection, lip proportion, jaw shape, skin thickness, pigmentation tendency, PIH history and scar tendency — plus individual aesthetic goals.
For skin-quality procedures and exosome-assisted treatments, inflammation and pigmentation risk are also considered.
THE GOAL IS TO SUPPORT YOUR FEATURES — NOT STANDARDISE THEM.
10Home Skincare
Skin quality built in the clinic is supported at home: gentle cleansing, hydration, barrier support and daily broad-spectrum SPF — with treatment actives introduced under guidance.
11FAQ
What is a biostimulator?
A treatment designed to encourage biological processes associated with collagen production, tissue quality, hydration or regeneration — rather than simply adding immediate volume.
How is a biostimulator different from dermal filler?
Dermal filler provides immediate structural support or volume. Biostimulators rely on a gradual response from your own tissues, so the effect develops over time.
Is Julaine the same as Sculptra?
Both sit within the PLLA category, but formulations, particle characteristics, preparation and protocols may differ. Beyond Skin UK uses Julaine — the confirmed clinic product.
Do biostimulators add volume?
Not like filler. Biofiller can provide subtle soft support; PLLA has an indirect progressive structural effect; exosomes are not a volumising treatment.
How many treatments will I need?
Biofiller — a course of 3 may be recommended, never guaranteed. PLLA — individualised. Exosomes — protocol dependent. All confirmed at consultation.
Are biostimulators suitable for Black and brown skin?
Yes — with an assessment-led approach that respects ethnic facial anatomy, pigmentation tendency and PIH history.
How much downtime is there?
Treatment dependent, confirmed according to the Beyond Skin protocol before anything is agreed.
